Summary
Introduction
The problem of intimate partner violence (IPV) in rural areas intersects with another rural crisis: the epidemic of opioid use disorder (OUD) and opioid-related deaths. Rural opioid-related mortality rates are particularly high in the New England region (Stopka et al, 2019). At the same time, feminist criminological research shows that rural women are more likely than their suburban or urban counterparts to experience violence when separating from or divorcing their spouses (DeKeseredy and Schwartz, 2009; Rennison et al, 2013). Rates of intimate partner homicide also appear to be higher in rural areas (Edwards, 2015). There is a known, bidirectional relationship between opioid use and IPV (Stone and Rothman, 2019), such that drug use may precede abuse by an intimate partner and experiencing IPV is related to subsequent drug use. Abusive partners may exercise control by introducing their partners to drugs or coercing them to use, or by interfering with a partner's treatment and recovery efforts (Warshaw et al, 2014). IPV survivors have also described how substance use in their relationships can increase violence, paranoia, jealousy and arguments over procurement or sharing of drugs (Gilbert et al, 2001).
By understanding the first-hand experiences of people accessing rural IPV and OUD services, we can identify gaps in service provision and work to close them. In 2020, this goal became even more urgent as the COVID-19 pandemic widened these gaps and highlighted limitations of social services to respond to rural communities in crisis. For example, the pandemic saw the rapid implementation and expansion of telehealth services (Demeke et al, 2020). Telehealth has the potential to expand access to care in rural communities and has demonstrated effectiveness and patient satisfaction (DelliFraine and Dansky, 2008; Kruse et al, 2017; Totten et al, 2019). However, it requires substantial investment in infrastructure (for both providers and communities), and uneven implementation may worsen rural health disparities (Hirko et al, 2020; Pierce and Stevermer, 2020).
In this chapter, we connect the results of two data collection efforts: (1) a series of in-depth interviews with rural Vermonters with lived experience of IPV and OUD regarding their experiences accessing services and resources; and (2) a series of in-depth interviews with representatives of a county-level Coordinated Community Response (CCR) team comprising professionals working in IPV-and substance use disorder (SUD)-related services.
Identifying opportunities for strengthening advice to enhance vegetable liking in the early years of life: qualitative consensus and triangulation methods

Objective:
To prioritise and refine a set of evidence-informed statements into advice messages to promote vegetable liking in early childhood, and to determine applicability for dissemination of advice to relevant audiences.
Design:A nominal group technique (NGT) workshop and a Delphi survey were conducted to prioritise and achieve consensus (≥70 % agreement) on thirty evidence-informed maternal (perinatal and lactation stage), infant (complementary feeding stage) and early years (family diet stage) vegetable-related advice messages. Messages were validated via triangulation analysis against the strength of evidence from an Umbrella review of strategies to increase children’s vegetable liking, and gaps in advice from a Desktop review of vegetable feeding advice.
Setting:Australia.
Participants:A purposeful sample of key stakeholders (NGT workshop, n 8 experts; Delphi survey, n 23 end users).
Results:Participant consensus identified the most highly ranked priority messages associated with the strategies of: ‘in-utero exposure’ (perinatal and lactation, n 56 points) and ‘vegetable variety’ (complementary feeding, n 97 points; family diet, n 139 points). Triangulation revealed two strategies (‘repeated exposure’ and ‘variety’) and their associated advice messages suitable for policy and practice, twelve for research and four for food industry.
Conclusions:Supported by national and state feeding guideline documents and resources, the advice messages relating to ‘repeated exposure’ and ‘variety’ to increase vegetable liking can be communicated to families and caregivers by healthcare practitioners. The food industry provides a vehicle for advice promotion and product development. Further research, where stronger evidence is needed, could further inform strategies for policy and practice, and food industry application.
Environmental enteric dysfunction and systemic inflammation predict reduced weight but not length gain in rural Bangladeshi children

Environmental enteric dysfunction (EED) and systemic inflammation (SI) are common in developing countries and may cause stunting. In Bangladesh, >40 % of preschool children are stunted, but EED and SI contributions are unknown. We aimed to determine the impact of EED and SI (assessed with multiple indicators) on growth in children (n 539) enrolled in a community-based randomised food supplementation trial in rural Bangladesh. EED was defined with faecal myeloperoxidase, α-1 antitrypsin and neopterin and serum endotoxin core antibody and glucagon-like peptide-2, consolidated into gut inflammation (GI) and permeability (GP) scores, and urinary lactulose:mannitol α-1 acid glycoprotein (AGP) characterised SI. Biomarker associations with anthropometry (15-, 18- and 24-month length-for-age (LAZ), weight-for-length (WLZ) and weight-for-age (WAZ) z scores) were examined in pairwise correlations and adjusted mixed-effects regressions. Stunting, wasting and underweight prevalence at 18 months were 45, 15 and 37 %, respectively, with elevated EED and SI markers common. EED and SI were not associated with 15–24-month length trajectory. Elevated (worse) GI and GP scores predicted reduced 18–24-month WLZ change (β −0·01 (se 0·00) z score/month for both). Elevated GP was also associated with reduced 15–18-month WLZ change (β −0·03 (se 0·01) z score/month) and greater 15-month WLZ (β 0·16 (se 0·05)). Higher AGP was associated with reduced prior and increased subsequent WLZ change (β −0·04 (se 0·01) and β 0·02 (se 0·00) z score/month for 15–18 and 18–24 months). The hypothesised link from EED to stunting was not observed in this sample of Bangladeshi 18-month-olds, but the effects of EED on constrained weight gain may have consequences for later linear growth or for other health and development outcomes.
